In Changes, is there a way to create a custom "template" sort of like you can in Service Catalog? Or is there a way to create a Service Catalog item that will generate a change request as well?
You can create custom forms and fields and tie them to the Change Request module, if that is what you are asking. We have done this to add a "Risk Level" and "Risk Detail" to our change request forms. You will find this under Setup > Custom Fields and Forms.
As for creating a Service Request that will generate a change request, there is no way to accomplish this today that I am aware of. Great idea though! We would definitely utilize that if it were possible.

One of our IT technical wizards created some code that created a change request off an incident.
