Approval task only has Approve or Deny but needs a Question option

When service requests are setup with a mandatory approval, and the approver asks a question, how can we generate a task back to other technicians without hitting approve or deny? The question is not directed to the requester but rather to other technicians associated with the service request. At this time, the task sits with the approver without others knowing that a question has been asked.
