New Form associated to Incidents (All, All)
Created a new form (+New Form) 'FCR Incident Test'
Created Custom Field (+New Field) 'First Call Resolution' Checkbox
Dragged the FCR Field into the newly created form
Part 2 Navigate to your service desk screen
Click on the blue wrench highlighted in my second screen shot
Select 'First Call Resolution' and now you will be able to see it on the main page.
