Be able to move customize fields and add fields that are currently in the user profile to the incidents form

I'd like to be able to require a contact method/phone number from users with the incident form. Including the user's number in the incedent.
You can always add this as custom fields. Under setup -> forms and fields.
Which I've done. The problem is I've nowhere to add this custom field to "New incident" form.
